From our ENDO 2026 series, PWN contributor and Assistant Professor of Medicine at UCSF, Dr. Francesca Galbiati, talks with Dr. Woun Seo, Senior Director of Medical Affairs at Eton Pharmaceuticals, about the liquid oral formulation of desmopressin recently launched under the trade name Desmoda by Eton Pharmaceuticals.
For decades, patients with arginine vasopressin deficiency (AVP-D, formerly known as central diabetes insipidus) who take oral desmopressin have had one basic format to work with: the tablet. And for many, the tablet has never quite fit. Getting to the right dose has often meant splitting tablets in half, or even quarters,to approximate what the body actually needs. The discussion centers on the benefits of starting with precise dose titration. A tablet of a fixed strength can be too much or too little, and splitting tablets is an imperfect fix. A liquid allows the dose to be dialed in far more exactly and adjusted as needs change. The second is patients who have difficulty swallowing pills — a real barrier for some, and one that a solution removes entirely.
Dr. Galbiati also raised a third scenario familiar to anyone who has been through pituitary surgery: the post-operative period. IV vasopressin has a very short half-life, minutes, not hours, which makes it appropriate only in intensive care settings. An oral solution, with a half-life measured in hours, could offer a practical way to manage water balance once a patient no longer needs an IV but still needs flexible, adjustable dosing.
